Online Patient Services
For access to your medical record and to view test results online, please complete the registration form in the link below and take to reception along with your photo identification in order to register your application.
Please note for security reasons you must prove your identity to Reception staff before we are able to supply you with an access code. You can only apply in person and once a patient reaches their 16th birthday they will be required to register themselves to comply with confidentiality regulations.
